i tried to help you but your questions are not very clear. if this is for an investigation you should fix them. like for example number four doesnt say if it is per day, week month etc...
 question number two..if a child drinks 2-3 per week the range goes to 8 per month  and your next range is 5-6 per month so it not a valid range for an investigation. just my thoughts

@morbeyaz: the way to get points is to upload worksheets that are attractive enough to interest people here.
There are many free tutorials that help you create the most wonderful worksheets, your students will love them and you will get lots of points here.
But what we don �t encourage is people begging for points- that would be like giving students good grades because they ask for them. ;-)
