You are on the train home means you are taking the train to get home  or you are on the way home by train.

You could say this if you answered your mobile phone while travelling home by train.
 
eg: I won �t be long now I �m on the train home.
 
But we would say I �m on the train to work.
 
I �m on the way home. (Not to home)
 
I �m on the way to work.
 
I caught/took a bus into town, but I caught/took a train home.
