I don �t see any problems with "me" and "I" sounds a bit strange but since you put "teacher" I think it would be more adequate to put "student" but that is just an opinion because that �s what I normally do.

Many forms say You but I think Me is clear and simple. Self is also an option. I wouldn �t put I.
