5. �you seem to be annoyed by me�.�(or �you seem to have been annoyed by me�, if you prefer to emphasize that the action has been completed)... Alternatively, you could use a modal verb of probability, such as �must�, �You must be/have been annoyed by me�...

For 4. I like Bruce �s answer, but I think you could also say. "This model stopped being used years ago."
For 11. I like the answer given, but one could also say, "I may not have remembered to give her your message."
