Give one sheet to each student. They write the correct form of the verbs given in the paranthesis first. And when everybody is ready, they go around the classroom and ask their friends whether they have done that activity or not. The one who finds a person for each gap is the winner. In all the classes I´ve applied this activity up to now, the students found it very enjoyable and it worked :)
