English worksheet: making questions, discussing interviews making questions, discussing interviews

I found that my students had problems with forming questions, especially in deciding when to use "Can you..." "Have you..." or "Do you..." With this in mind, I made this worksheet containing a selection of sentence fragments, such as "ate ice cream yesterday" or "is good at basketball", and it becomes the students task to make questions of them. For example, "Did you eat ice cream yesterday?" or "Are you good at basketball?" Following on from this is a work-sheet based around the theme of ´interviews´; in this case, an interview for one entering high-school.
