English worksheet: Book Worm Book Worm

In my school we are fortunate to have a decently-sized English library and so I sought to make use of it in class. To this end I designed a worksheet that is easy to use and unintimidating. Students choose 2 books to read, after reading they are told to fill out their worksheet and describe their book. The adjectives in the word bank are ALL reviewed thoroughly beforehand. The student then rates the book they read as EASY MEDIUM or HARD (difficulty level)
