Only my experience:
I always devote the first class to get to know the students, their needs, knowledge, experience with English, likes and dislikes and their expectations.
After listening I tell them about the way we are going to work and my own expectations.
If some time left, time to start the real thing with some game or song. If I make it interesting, I will have caught their interest and attention.
