This is a test I have used for my students and it includes PRESENT PERFECT, FOR or SINCE, CONDITIONAL TYPE 1 and LIST OF IRREGULAR VERBS (INFINITIVE, PAST SIMPLE, PAST PARTICIPLE and TRANSLATION)

THis is another option for the previous test. Just in case you have two different groups or if you want your students to retake the test. The list of verbs is the same, but the activities are different.
