Hi mad_rdg!
This is a type of exercise to make students work with word forms.
The root is "care", which is a verb. "Careless" is an adjective made up of the verb "care" followed by the suffix "less" meaning without. To make it an adverb, you can add the suffix "ly" to many adjectives, therefore constructing "carelessly". "Ness" is the suffix added to many adjectives as well, but this time to create a noun (carelessness).
Because of the structure of this sentence, "entirely to her own..." the possessive adjective "her" shows you need a noun after it, and as the only noun in the multiple choice is D, then that s the answer!
Cheers!

Here is small tip to help you distinguish them.
Look at the gap. If it can be filled with the word "what" then what you are missing is the noun:
it was due to her own....(what)?... carelessness (noun)

if the gap can be filled with "what kind" you are missing an adjective
it was due to her.......(what kind) ....actions.   careless (adjective)

if the gap can be successfully filled with "how" you are missing the adverb
it was due to the fact she was acting ..............(how?) ....  carelessly

if the verb is missing the gap can be filled with "what did he do"
she .............what did she do ............carelessly    acted (verb)

Hope it helps you tell them apart
