In the first question, it depends on the context. They could all be correct depending on who you are referring to. Without a context, it is impossible to say which is correct.

Also, for the second question, I am not sure what you mean by "broken order" but they are all common collocations except "fish and cheese". However, fish and cheese is not a "broken order" or wrong, it is just an unusual collocation.

"I hate fish and cheese." Nothing wrong here but it isn �t a common collocation which is I think what you are trying to get at?

If they have the same father, then it �s "Jack and Jill �s father"
 
If they have different fathers, then either "Jack and Jill �s fathers" or "Jack �s and Jill �s fathers" is correct.
 
I can �t think of a situation in which "Jack �s and Jill �s father" would be used.
 
Like freddie, I don �t know what you mean by "broken order".

I �m guessing that what you mean by broken order, is something to do with not traditionally fitting together--in that case fish and cheese would be your "broken order" (all the others are traditionally seen as belonging together)--which Freddie probably rightfully called collocations.
 
For my US friends: Bangers and Mash "is a traditional British Isles dish made of mashed potatoes and sausages" and  is also known as Sausage and Mash.

Bruce is right about the first question. 
Yes, on the second one, �fish and cheese � don �t go together, although they do taste nice together. I would add that we always say �cheese and wine � rather than �wine and cheese. �
