- Supervisor�s evaluation         means the supervisor evaluate me. 
 
- Teachers� evaluation               means the teachers evaluate me.

Yes. The supervisor�s evaluation is the evaluation/judgement/ marks the supervisor gives.
The teachers� evaluation is the evaluation of the teachers.  
Edit: Douglas makes a good point: without any mention of a person, that could be right.

As I see it, it could mean either one: the supervisor/teacher is the evaluator or the evaluated
