Doctor Office Form for Patient Intake Activity
This is an activity sheet for students to work with health vocabulary and practice filling out patient intake forms at a doctor´s office or urgent care. It is great for use after a vocabulary lesson on symptoms, insurance, etc.
I had the students write a journal about the time in their life when they were the "most sick," then asked them to pretend they were back in time at the doctor´s office. They worked in pairs to complete the forms - one student filling it out while interviewing the other student. It was very interesting and educational!
