I give this questionnaire to my students on our first lesson. There are several personal questions like "What´s your name?" or "Have you got a pet?". Students work in paris, first putting info about themselves and then asking their partners. You can add more columns, so your students ask more than one person. It´a a great icebreaker when the group meets for the first time and doesn´t know each other.
