How to write an address as regards living in a block of flat

Hello fellow teachers
Block of flats are extremely popular in Poland.I am wondering  how to write  somebody s address. We write such addresses this way :
Klonowa 12/2
01-051 Warsaw
woj. mazowieckie

(street s name ) Klonowa  12/2, where 12 stands for the number of  the block of flat in which somebody lives, whereas  2  means the number of  flat
01-051 Warsaw   00-051  is  the  zip code
woj. mazowieckie  means region
I have come across a stange word in a dictionary , namely voiviodship. I guess that such a word doesn t exist. Am I right ?
Do you read  12/2 as slash  ? Do you pronounce 0 in  01-051  as zero ?
How do you pronounce the number of a bus, e.g.  302  three hundred and two  or three zero two  ?
Thanks for your help. HAND

USA:

325 Short Steet Apt. 3a
Newton, CA 93444

325= street number
apt. 3a = apartment number
Newton= town/city
CA= state code (California in this case)
93444= zip (post) code

I also see in the USA apartment addresses like this:

119-D Eugene Street
Knoxville, TN 37917

The D is the apartment number in this case, and the state is Tennessee. This, of course, is a made-up address, although this is a ZIP code used in Knoxville. Someone else asked the same question a few weeks ago, but I couldnt find the thread.

Australia:

2/12 Klonowa
WARSAW   01-051   WOJ MAZOWIECKIE

or

Flat 2, 12 Klonowa
WARSAW   01-051   WOJ MAZOWIECKIE

"voiviodship seems to be the Polish word for province

Do you read  12/2 as slash? - for an address where the flat no is 2 and the street no is 12 we we usually just say "two, twelve Klonowa" or "unit two, twelve Klonowa"

Do you pronounce 0 in  01-051  as zero ? We usually say "oh one oh five one" but some people do say zero instead

How do you pronounce the number of a bus, e.g.  302  three hundred and two  or three zero two  ? - "three oh two"

Moody - the previous thread is here

Hope this helps

Cheers
